Continuum of Care at Sioux Center Health Senior Living
Sioux Center Health’s Senior Living community is a not-for-profit community designed to meet the needs of seniors throughout the aging journey. We offer three distinct levels of service – independent living, assisted living, and long-term care – and a host of services to ensure comfort and security. Our Senior Living community offers maintenance-free living and is designed to enhance independence and well-being.
Our objectives are based upon the needs and desires of our community and upon a long-term commitment to our mission. As a not-for-profit organization, our focus is on those we serve. Revenues generated through our services are reinvested for the betterment of our seniors and community’s health care needs.
Crown Pointe – Independent and Assisting Living
Bordering the countryside in the southeast corner of Sioux Center, Crown Pointe focuses on the importance of tenant security, safety, wellness, health education, and socialization. The amenities include a spacious community room for family gatherings, comfortable dining room and lounges, library with computers and books on loan through our partnership with the Sioux Center Public Library, barber/beauty shop, exercise room, billiards room, activities room, ice cream parlor, and a beautiful outdoor patio.
Crown Pointe is located on a beautifully landscaped, 11-acre campus and offers a home-like retirement community for seniors. In addition to the beautiful facility, Crown Pointe staff members are dedicated to meeting your needs and ensuring you have a wonderful place to call home. More
Franken Manor – Independent and Assisting Living
Franken Manor is conveniently located downtown, near the business district of Sioux Center and is a senior living community that promotes tenant independence, security, safety, socialization and well-being. The amenities include a large dining room that also serves as a shared community room for socialization, a barber/beauty shop, an activities room, and an outdoor patio with views of highway 75.
Franken Manor is one of the few assisted living facilities in Iowa that accepts payment through Iowa Medicaid and U.S. Department of Housing and Urban Development (HUD). These programs can assist qualified seniors with rent and personal care services. More
Royale Meadows – Long-Term Care
Royale Meadows is a state-of-the-art, skilled nursing facility. Our four neighborhoods are designed to provide comfort and quality care to all those who call Royale Meadows home. A key feature of the beautiful nursing home facility is the spacious community rooms designed to accommodate all sizes of families. Royale Meadows care center is designed as a homelike atmosphere where staff members focus on resident-centered care. Family members and friends of residents are encouraged to come as often as they can. Royale Meadows staff’s desire is to make life as comfortable and homelike as possible while meeting the special needs of each resident.
The amenities at Royale Meadows include spacious community rooms, barber/beauty shops, outdoor patios, transportation services, activities programming, dietary services for resident and guests and much more.
Home Care is a coordinated program of skilled services for patients and their families in the home setting. Our nurses, home health aides, therapists, and social workers are trained to serve recovering, disabled, and chronically ill persons of all ages. Private Pay Home Health Care Services
Private pay home health care services allows people the freedom to stay in the comfort of their own home while receiving necessary health care and environmental assistance. All care is supervised by registered nurses. An initial nursing assessment is performed to set up an individualized service plan based on personal needs. More
Our hospice program offers coordinated services to terminally ill patients and their families in their chosen environment. Physical, psychological, social, and spiritual care is available from a medically directed interdisciplinary team consisting of physicians, nurses, therapists, home health aides, social workers, pastors, and volunteers.
