Program Details
In-home health care allows people the freedom to stay in the comfort of their own home while receiving necessary health care and environmental assistance. The Sioux Center Health Hospice and Home Care staff is trained to serve recovering, disabled, and chronically ill patients of all ages. All care is supervised by registered nurses. An initial nursing assessment is performed to set up an individualized service plan based on personal needs.
In-Home Care Objectives
Personal Care and Homemaking Services provided by Certified Home Care Aides
Services may include, but are not limited to:
- Assistance with bath or shower
- Dressing and undressing
- Care for hair and nails
- Ambulation and exercises
- Essential shopping and errands
- Dust and vacuum
- Companionship
- Clean bathroom
- Clean kitchen and dishes
- Wash laundry and bedding
- Wash interior windows
- Plan and prepare meals
- Empty garbage
Nursing Care Provided by Registered Nurses
- Vital sign monitoring
- Medication management
- Weight monitoring
- Venipuncture
- Education on disease management
- Health maintenance visit every 60 days to access current and future needs
Admissions and Reimbursement
Referrals are accepted from family members, friends, hospitals, physicians, or the person in need of service. All services are coordinated with your physician and are provided to patients residing within a thirty-mile radius of the Sioux Center Health Hospice and Home Care office. We do not discriminate against age, race, religion, sex, nationality, color, or handicap.
Most services are provided on a private-pay basis. Financial assistance is available based on need. This program accepts grants through Community Public Health to assist those with difficulty to pay. Applications are completed on admission.
