We recommend calling ahead whenever possible to make an appointment. You can also request an appointment through AveraChart.
If you need to be seen the same day and your regular healthcare provider is not available, we will offer you an appointment with one of our other providers.

Your Medical Information
As a new patient, you will be asked to provide information about your medical history. The information in your chart is confidential, and the entire staff will protect that confidentiality. If you wish to release your records for any
reason other than for continuing care or billing purposes, you will need to sign an authorization to release medical information. We will send the information to you. Records Release Information.
If You Have a Question
If questions arise regarding your care and you would like to talk to a nurse during office hours, please call us. A receptionist will take your name, phone number, and reason for calling. The nurse will work with your physician to answer your question and advise.
Prescription Renewals
Please contact your pharmacy for a prescription refill. Sometimes, your prescriptions can be refilled without calling your physician. The pharmacy will handle getting your prescription. If you have medication that cannot be refilled without returning to the provider, please make an appointment before your medications run out. Always bring your medications or a list of them when you come for an appointment.
If You Have an Emergency
For an emergency during office hours, please call the office and state you have an emergency. The nurse will speak with you immediately and give you further instructions.
After office hours, a provider is on call at the hospital. You can call the hospital at (712) 722-1271. If you need to be seen by the provider on call, you will be directed to go to the hospital’s emergency department.

Your Financial Responsibility
At each visit we will ask you to verify your insurance information, home address, and telephone number.
- We will ask you to show your insurance card at each appointment.
- You will need to pay your co-pay or co-insurance at each appointment
(this is a contractual agreement you have made with your insurance company). - If you do not have insurance coverage, payment in full is expected at each
appointment. Payments may be made in the form of cash, check, Visa,
Mastercard, or Discover.
If you have a question regarding your Medicare coverage, call (712) 722-8252. For all other insurance or Medicaid questions, call us at (712) 722-8435 and an insurance specialist will assist you.
