Over 70 years ago, a group of local leaders from Sioux Center envisioned a community hospital to serve the health needs of our community. In 1951 that vision became a reality when the new hospital building was constructed and the doors were opened.

  • 1951: Sioux Center Community Hospital Opened
  • 1972: OB/ER/Lab Addition
  • 1985: Hospital Replacement
  • 1986: Outpatient Clinic Opened
  • 1998: Remodeled OB/ER/Lab
  • 2004: Purchased Sioux Center and Hull Medical Clinics
  • 2009: Purchased Land for Replacement Facility
  • 2014: Sioux Center Health Opened New Facility
  • 2019: Purchased Hawarden Medical Clinic
  • 2021: Expanded Sioux Center Medical Clinic

In 2014, Sioux Center Health opened a new 123,500 square feet facility that provides family-centered care throughout all stages of life. The beautifully designed facility was based on Psalm 23 that gives patients a healing environment for maximum patient safety, privacy, and comfort. The facility houses many departments such as the medical clinic, specialty clinic, surgery center, emergency, therapy center, inpatient care, and more.

The hospital is a 19-bed acute care facility with a “swing bed” (skilled nursing) program for a continued stay after being dismissed from acute care. Through a partnership with Avera, electronic ICU is provided for intensive care monitoring so patients can stay close to home while receiving around-the-clock care.

Our three medical clinics in Sioux Center, Hawarden, and Hull feature experienced family medicine providers who care for patients with health concerns or routine checkups. The hospital continues to offer a variety of specialty clinics and outreach services allowing patients to stay close to home.

The Birthing Center offers expectant mothers the total comfort of home and peace of mind. The Birthing Center features two spacious birthing rooms, four postpartum suites, whirlpool spas, wireless internet, and comfortable family rooms and snack bar.

For local area seniors, the organization provides a full continuum of senior living facilities. Crown Pointe Estates includes a 69-bed skilled nursing facility, provides 24-hour nursing care in a newer state-of-the-art homelike setting. Crown Pointe Estates Independent/Assisted Living facility offers a variety of services that promote independence and choice in a supervised and caring environment.

As the town continues to grow, so do the health needs of the community. Sioux Center Health remains involved in community education and support programs, physical therapy, child daycare, occupational health services, community health screenings, cardiac rehabilitation, memory diagnostics, and an extensive home health and hospice program.

Community Health Needs Assesment (CHNA)

The Community Health Needs Assessment (CHNA) is a federal requirement for 501(c)(3) hospitals once every three taxable years. The information must be disclosed in its annual information report to the IRS (i.e., Form 990 and related schedules). While the CHNA is a federal requirement, we must remember that assessing our communities and providing community benefit programs are more than a nonprofit obligation. Conducting a needs assessment and responding to those community health needs allows us to live our mission and make a significant difference in the communities we serve.
